Al registrarse a jugar con GGSL usted estará reconociendo y aceptando que está jugando bajo su propio riesgo. GGSL no se hará responsable por algún robo o lesiones que puedan ocurrir dentro las horas de operación. Seguro Deportivo está disponible para jugadores de 16-18 años de edad; se aplican exclusiones, consulte al director para obtener más detalles. Jugadores de 18 años y abajo deben tener la firma de un padre o tutor.
GGSL tiene el derecho de rechazar formularios de registración de quienes no se comporten con buena deportividad.
La información proporcionada en este formulario se mantendrá confidencial y solo se utilizará para asuntos relacionados con la liga. Al firmar, usted acepta seguir las reglas y regulaciones que GGSL tiene vigentes.

INFORMED CONSENT:
By registering to play in GGSL you are acknowledging and agreeing that you are playing at your own risk. GGSL will not be liable for any theft or injuries that may occur within hours of operation. Sports insurance is available for players 16-18 years of age, exclusions apply, see director for more details. Players 18yrs. and under must have a parent or guardian signature.
GGSL has the right to reject registration forms of those who do not conduct themselves with good sportsmanship.
